A printed scarf/wrap and crazy beaded earrings and/or bright shoes (::cough:: my dark red boots ::cough::) is a good default too. Overall though I think the material of the tshirt and the color of the demin is really important. I feel more dressed up in darker denim and a t - even if it is cotton with slightly more stretch (think it hangs better.) I do the addition of a blazer as well. I got it a few years ago and I still wear it, often with a t-shirt and jeans as well. I work in a pretty formal environment but I like to think that "a blazer covers over a multitude of jeans." ...
